The guide says the lvl range is 150-220, but isnt it 150-200? (or 150-199)?

Solo instance is to 220. Team is to 200.

not sure what the bottom level for entry is, but I go into DR with 120+ lvl chars to solo the crawlers for very good easy xp when teams are sparse

you cannot pull the quest though, that is limited to 150 plus.

That is correct, Level 150 is the minimum level you can pull the quest, and since this is a 'Quest Guide', that's why it's been labled 150-220.

We could add to the quest guide that you are able to enter but not pull the mission. People look at our guides for information about Dark Ruins generally
